Purpose: The user can add (custom) fields (e.g. route) to an item type (e.g. vaccines), and the user can create new items (e.g. polio vaccine) that inherit from this item type. This is to edit the draft item type (aka HM category). In order to use this template the user needs to push this template to the user repository.

To add a new item:

  1. Click [Add Item].

  2. The system will prompt you to enter the name for the new item. Enter the name for the item and click the “OK” button.

  3. The system will prompt you to set a priority for this item. The default priority is 4. Enter the priority you wish to give to this item and click the “OK” button.

 

To add a new field:

  1. Enter the name for the field you wish to add.

  2. In the “Position” dropdown, selected the position you wish to place this field.

  3. In the “Type” dropdown, selected the type you wish to use for this field.

  4. Click the “Submit”. To add another field, repeat these steps.

  5. Click the “Save” button to save this template.

Note: After creating the HM template. The template can be appended to the PHM section of the patient’s office note. The user can select the template from the “Add HM Item” dropdown.
