Overall function, use & design

This page allows you to record insurance or other entity verification.

1. Choose the policy.
   
2. Complete all the data fields as necessary.
   
3. You may add copay per CPT code in this screen:
   
   • First, click on "Search & Add CPT/Copay".
     
   • Select CPT code(s) and drop them into the "Selected CPT" box by highlighting the items and clicking on the blue arrow.
     
   • Finally, click on "Close & Add CPTs".
     
   • Fill in the amount of the copay for that code
     
4. Next fill in the deductible and eligibility information.
   
5. Finally, click on the "Save" button.
